سلامت اجتماعی
برای مفهوم سلامت اجتماعی [1] می­توان دو سطح در نظر گرفت. یکی «سطح کلان» که دربر دارنده­ی شاخص­های سلامت جامعه است و در این معنا سلامت اجتماعی را می توان معادل «جامعه ی سالم» در نظر گرفت که در آن شاخص­هایی چون میزان فقر، آموزش، جرم، آلودگی های زیست محیطی، آزادی­های مدنی و غیره را می توان برای کل جامعه در نظر گرفت و سطح دیگر، «سطح خرد» است که به بررسی سلامت اجتماعی فردی با شاخص­هایی چون میزان ارتباطات و تعاملات فردی، مهارت های اجتماعی، سازگاری، انطباق با محیط و غیره می پردازد. سطح اول به سلامت اجتماعی جامعه و دومی به سلامت اجتماعی افراد نظر دارد.

مفهوم سلامت اجتماعی فردی

مفهوم سلامت اجتماعی، مفهومی است که در کنار ابعاد جسمی و روانی سلامت، مورد توجه قرار گرفته است. بلوک و برسلو[2] (1971) برای اولین بار در پژوهشی به مفهوم سلامت اجتماعی می پردازند. آن ها سلامت اجتماعی را با «درجه ی عملکرد اعضای جامعه» مترادف کرده و شاخص سلامت اجتماعی را ساختند. آن ها تلاش کردند تا با طرح پرسش های گوناگون در ابعاد جسمی، روانی، اجتماعی سلامتی فردی، به میزان فعالیت و عملکرد فرد در جامعه برسند.

این مفهوم را چند سال بعد دونالد[3] و همکارانش در سال 1978 مطرح کردند و استدلال آن ها این بود که سلامت، امری فراتر از گزارش علایم بیماری، میزان بیماری ها و قابلیت های کارکردی فرد است. آن ها معتقد بودند که رفاه و آسایش فردی امری متمایز از سلامت جسمی و روانی است. بر اساس برداشت آن­ها سلامت اجتماعی در حقیقت هم بخشی از ارکان وضع سلامت محسوب می شود و هم می­تواند تابعی از آن باشد (به نقل از امینی رارانی، 1389).

طبق نظر کییز[4]، حلقه­ی مفقوده در تاریخچه­ی مطالعات مربوط به سلامت، پاسخ به این سؤال است: آیا ممکن است افراد، کیفیت زندگی و عملکرد شخصی خود را بدون توجه به معیارهای اجتماعی، ارزیابی کنند؟ کییز در پاسخ به این سؤال، مفهوم سلامت اجتماعی را به عنوان یکی از ابعاد سلامت مطرح می­کند. طبق تعریف وی سلامت اجتماعی عبارت است از: ارزیابی و شناخت فرد از چگونگی عملکردش در اجتماع و کیفیت روابطش با افراد دیگر، نزدیکان و گروه­های اجتماعی که عضو آن­هاست (کییز، 2004).

لارسن[5] (1996) سلامت اجتماعی را به عنوان ارزیابی فرد از کیفیت روابطش با خانواده، دیگران و گروه­های اجتماعی تعریف می­کند و معتقد است که مقیاس سلامت اجتماعی، بخشی از سلامت فرد را می سنجد که نشانگر رضایت یا نارضایتی فرد از زندگی و محیط اجتماعی است و در واقع شامل پاسخ های درونی فرد (احساس، تفکر و رفتار) می­شود.

به طور ضمنی، مفهوم سلامت اجتماعی کمتر شبیه به ابعاد جسمی و روانی سلامت است؛ اما در کنار سلامت جسمی و روانی، این نوع از سلامت نیز یکی از سه رکن اساسی اکثر تعریف های سلامتی است. تا حدودی این بدین دلیل است که سلامت اجتماعی هم به ویژگی های جامعه بر می گردد و هم به خصوصیات افراد. جامعه زمانی سالم است که فرصت و دسترسی برابر برای همه به خدمات و کالاهای اساسی وجود داشته باشد تا افراد جامعه به عنوان یک شهروند، عملکرد کاملی داشته باشند. نشانگرهای سلامت جامعه، ممکن است شامل عمل به قانون، برابری در ثروت، مشارکت در تصمیم گیری و سطح سرمایه ی اجتماعی باشد. سلامت اجتماعی افراد به سطح رفاه و آسایش در رابطه با چگونگی بودن در کنار دیگران، چگونگی واکنش افراد دیگر با فرد و چگونگی تعامل فرد با نهادهای اجتماعی و آداب و رسوم اجتماعی اشاره دارد.

مطالعه­ی مربوط به سلامت با استفاده از مدل­های زیستی، بیشتر بر وجه خصوصی از سلامت تأکید دارند؛ ولی افراد در درون ساختارهای اجتماعی و ارتباطات، قرار دارند و با چالش ها و تکالیف اجتماعی بی شماری رو به رو هستند. کییز (2004) معتقد است بهداشت روانی، کیفیت زندگی و عملکرد شخصی فرد را نمی­توان بدون توجه به معیارهای اجتماعی، ارزیابی کرد و عملکرد خوب در زندگی چیزی بیش از سلامت روانی، هیجانی و شامل تکالیف و چالش­های اجتماعی است.

سلامت هیجانی بیانگر رضایت و عاطفه­ی مثبت فرد در ارتباط با زندگی به معنای کلی (نه فقط زندگی اجتماعی) است. ابعاد سلامت روانی یک بازتاب درونی از سازگاری فرد و دیدگاهش نسبت به زندگی را بیان می­کند. تنها یکی از 6 بعد سلامت روان (روابط مثبت با دیگران)، بیانگر توانایی برقراری و حفظ روابط صمیمی و اطمینان بخش بین فردی است؛ لذا در حالی که سلامت روانی و هیجانی بیانگر بعد خصوصی و شخصی ارزیابی های عملکرد فرد است، سلامت اجتماعی بیشتر به ابعاد اجتماعی و عمومی که افراد توسط آن عملکردشان را در زندگی ارزیابی می کنند، توجه دارد. ابعاد سلامت اجتماعی در مقایسه با بهداشت روانی و هیجانی، ممکن است کمتر بیانگر بهداشت روانی فرد باشند؛ اما تعریف کلی سازمان بهداشت آمریکا از بهداشت روانی، شامل ابعاد خاصی از قبیل فعالیت های پربار، روابط بالنده و توان انطباق با تغییرات می شود که همگی بیانگر ارتباط کامل فرد با اجتماع و زندگی اطرافش است (کییز، 2004).

از نظر تجربی، مفهوم سلامت اجتماعی ریشه در ادبیات جامعه شناسی ناهنجاری و بیگانگی اجتماعی دارد، اما مطابق مدل سلامت، نبود ناهنجاری و بیگانگی اجتماعی، دلیلی کافی برای وجود سلامت اجتماعی جامعه نیست. کییز (1998) در یک تحلیل عاملی گسترده یک مدل 5 بعدی قابل سنجش از سلامت اجتماعی ارائه داد. طبق این ابعاد، فرد سالم از نظر اجتماعی زمانی عملکرد خوبی دارد که اجتماع را به صورت یک مجموعه ی معنادار، قابل فهم و دارای نیروهای بالقوه برای رشد و شکوفایی بداند. احساس کند که متعلق به گروه های اجتماعی است و خود را در اجتماع و پیشرفت آن شریک بداند. محتوای مقیاس سلامت اجتماعی، طیف مثبتی از ارزیابی تجارب فرد در جامعه است. این مقیاس جدید با مقیاس های سلامت اجتماعی بین فردی (مثل پرخاشگری و حمایت اجتماعی) و سطح اجتماعی (مثل فقر و جایگاه اجتماعی) تفاوت دارد.
